Purpose of Collection: The information collected in this online form will be used for following purpose:

-We will assess your qualifications as a potential committee member to assure that CSA's committees contain a fair balance of interests.
-If you are placed on a committee, the contact information provided such as your name, organization, address, email, telephone and fax numbers will be shared with other members (and their constituency) of the same committee, including Committee Information Subscribers, to facilitate the sharing of relevant technical committee information.
-The information will be used within CSA for the purpose of providing member benefits and other related information.

Disclosure: No information will be rented, tracked, given or sold to any third parties, unless necessary for the purpose of sending mass communications via a third-party supplier. In this case, CSA will have confidentiality agreements with its supplier.
